 Right Click > Save Image As (On The Smiley Preview Image).
When in an instant messaging conversation, click 'Select an Emoticon.
Near the Your Emoticons header, click 'Show All'
On the right, click the create button
Finally, select the smiley image file, assign a key and thats it!
